Application Package Checklist

Before mailing your application package, please ensure that the following items have been filled out completely and included in your package.

• Application Form duly filled out.
  
  
• Payment Authorization Form and cheque or money order, if applicable.
  
  
• TOEFL results  (if applicable).
  
  
• Any additional documents requested by your program or faculty.
  
  
  • Letters of Recommendation
  • Curriculum Vitae
  • Research Proposal or Statement of Interest
  • Autobiographical Questionnaire
    
    
• Ensure any Official Transcripts and/or letters of recommendation have been sent directly to Saint Paul University.  (If documents cannot be sent directly, they should be placed in a sealed envelope, signed across the flap, and included in your package).
